Root Canal Disinfectants, Antibiotics Prove Inadequate to Eradicate Bacteria

The bacteria dentists use to determine whether treatment of a root canal infection has been successful, namely the absence of discomfort and pain; x-ray evidence that abscessed areas, granulomas and cysts have completely filled in with new bone; and trust that the medicaments used in producing these results completely eradicated bacteria, all proved inadequate when such teeth were subjected to further testing by Dr. Price and his group.

Testing for the preseence of bacteria in root canal, after the use of disinfectants which appeared to have controlled the infections, proved that the organisms had not been eradicated.

Dr. Price, in making cultures after the use of over 100 different medicaments, including different strengths of each, found only eight which had negative responses after 24 hours, and only two after 48 hours of incubation; longer periods were found to be ineffective as medicaments lost their disinfecting power. root canal fillings

The two most efficient medicaments proved to be objectionable – silver nitrate because it turned teeth black and formalin because it was very irritating and painful to patients and destructive to surrounding tissues. 

Though culturing to determine whether bacteria had been killed was formerly taught and practiced, I know of no dentists using such a testing procedure today.

Dr. Price in another series of tests took extracted infected teeth and treated them (much easier to do aseptically outside of the mouth) and found when these teeth were sectioned their dentin and cementum were still infected.

In still another investigation, Dr. Price made biopsy sections at what is root canaland below the gum line and found bacteria present not only in the root of the teeth and their protective cementum but also in the first millimeter or two of adjacent bone. The importance of this factor in causing cavitations in bone after extractions will be discussed in a later chapter.

Dr. Price noted future developments might bring forth medicaments which are more successful. You will all be thinking that antibiotics are certainly the answer. However, antibiotics have not proven able to effectively penetrate the dentin tubules, cementum and adjacent bone sufficiently to eradicate these bacteria.

Testing of new medicaments, and current efforts such as new laser treatment methods, are of course necessary and were anticipated by Dr. Price.

All of these root canal side-effect problems should not deter the profession from continuing to explore new methods of how to save teeth. While we cannot learn how to save teeth by extracting them, Dr. Price fortunately has shown us ways to test procedures to prove whether infection has been eradicated in teeth we think we have saved.

Were Dental Implants found in Egyptian and Mayan cultures?

Indeed, continuing dental education has a long history.

Dentistry has a long history with evidence of dental treatment on skulls in the time of the pharaohs. More specifically, there is evidence of successful dental Implantology on a Mayan skull from the time 600 A.D. Pieces of shell are implanted in the lower front tooth redental implants, cost of dental implantsgion, which appear to have fused with the bone. This evidence would indicate that the treatment was successfully completed on a living human being. The artifact is now in the Peabody Museum of Harvard University. Today, dental implants are made of an alloy of titanium. This is the same alloy used for other prosthetic joints in the body. It is used because of its biocompatibility. There are no reported cases of allergy or rejection of the titanium alloy, and it is non-carcinogenic.

Early pioneers in modern dentistry did meet with some limited success in Implantology; however, it is the period from the 1960’s to the 1980’s where dental implants became a treatment that all dentists could recommend to their patients with confidence. Today’s dental Implantology offers patients a way to replace one or more missing teeth with an artificial root shaped anchor in the bone, and an artificial crown. Success rates for dental implants are very high, approaching 100% in some studies. Of course, each patient’s particular need is different, and the complexity of the treatment is often related to the amount of success one can expect. Patients who have severe systemic disease, smoke heavily, or have a poor jaw bone site for implantation may expect lower success rates. To find out if you are a candidate you must seek a consultation with a dentist who has training in this specialized field of dentistry. Currently, most dentists are involved with dental implant treatment, but not all. If you are going to lose a tooth, dental implants should be considered. Even if your dentist dismisses dental implants, request a referral to a dentist who commonly does this type of treatment for evaluation of your mouth.

Dental implants have a few distinct advantages over conventional dental prosthesis; specifically, fixed bridges and removable dentures. For thostooth replacement, implantology, dental educatione who are missing all their teeth and suffer with complete dentures, two to four implants can help to stabilize a loose denture. This type of treatment has been shown to help improve the health, nutrition, and quality of life for those who suffer with complete lower dentures. For others who are missing only one tooth, it offers the advantage of being able to restore that missing part of the body without involvement of the surrounding teeth. Finally, there are those who are missing teeth only in the back of their mouth, and have no adjacent teeth to use as a support for a fixed bridge. A removable prosthesis is usually cumbersome and difficult to get used to, and, has a great psychological barrier. In all of the cases mentioned, dental implants are the treatment of choice, and, in fact, are considered by most to be the current standard of care.

The process of getting dental implants varies from case to case, as does the time to complete the treatment. The basics of the procedure consist of the dentist first evaluating if you are able to get an implant with a proper examination. This will always include proper x-rays and often a CT scan of the jaws. A CT scan allows the dentist to view the area where the implant is to be placed in three-dimensions. Conventional x-rays only allow a two-dimensional view and distort the image to some degree. Placement of a dental implant is a surgical procedure and there are many different techniques to accomplish this. Usually, the dentist will make an incision in the gums to access the bone underneath. The bone is gently prepared to accept the implant and it is usually screwed into a precisely fitting hole.

The implant usually requires a period of undisturbed healing of 3-6 months. During this time the implant is buried under the gums and not visible. After healing the doctor will access the top of the implant again to place a cap on it that will protrude through the gums. The final steps are the taking of dental impressions and fabrication of the artificial post and tooth that will all be anchored by the implant. Even though the previous description might make you wince, it is usually easier than you think. The pain experienced is usually compared to a routine tooth extraction. During the procedure the dentist will anesthetize the area with a local anesthetic, just like when you have a filling. For more complex cases, patients may be sedated in the doctor’s office, or hospitalized for general anesthesia in the operating room. But, the overwhelming majority of cases are done with routine local anesthesia.

Some newer techniques offer patients the opportunity to extract a tooth that is hopeless and place the implant immediately into the extraction socket. Sometimes, these implants may be loaded with a temporary tooth at the same time. In the same way, some patients who will unfortunately loose, or have lost, all of their teeth may have implants placed and a prosthesis placed immediately. This type of prosthesis is fixed in place and offers the stability and secure feeling of natural teeth. Other patients are not able to have their implant and their new tooth attached immediately. In these cases other types of temporary teeth are fabricated. There are many types of these temporary teeth; usually the patient and doctor decide together what the best choice is.

As in all things, patients need to be good consumers in their choice of dental care too. Dental implants are state of the art and offer patients many advantages over previous techniques. Anyone who is missing a tooth, or who needs to have a tooth extracted, should inquire about dental implants.

Five types of Dental Implants

There are 5 basic types of dental implants available and the type chosen for a particular patient depends upon a number of factors. The size and condition of the patient’s natural jawbone is probably the main factor and of course no two patients are the same. Some jawbones can be wide and deep and others narrow and shallow with many variations in between.

The most commonly used material used for implants is the inert metal titanium because it has been found to fuse well with live bone tissue in the jawbone. This fusion process is known as ‘osseointegration’. The highly skilled work of inserting implants is carried out by a perodontist or oral surgeon who operates from within a hospital environment or more likely from a private clinic.


Root Form Implants

As the name suggests, root form implants resemble the root of a tooth but titanium dental implantsare formed like a self-tapping screw which gives it a good surface area for maximum fusion to the living bone. It is inserted by screwing the implant into a hole previously drilled into the jawbone or into the cavity left from an extraction. The implant then acts as a root or foundation for a new prosthetic crown. However, this type of implant can only be used if there is sufficient width and depth to the jawbone. Sometimes it is necessary to graft additional bone to the jawbone before dental implants can be considered.

Plate Form Implants

If there is insufficient live bone structure because the jawbone is too narrow or not deep enough then plate form implants is another option the perodontist will consider. This takes the form of a long narrow strip of titanium which is inserted between the jawbone and the gum and will fuse with the jawbone giving a foundation for a number of new prosthetic crowns if required. Sometimes it may also be necessary to graft additional bone if the jawbone is in a poor condition before the implant can be inserted.

Subperiosteal Implants

Subperiosteal implants are an option used where the condition of the jawbone is such that an insert is not possible and a bone graft is not feasible. The jawbone is exposed and a mould impression taken from which the subperiosteal implant is created and placed on top of the jawbone under the gum and fusion takes place between the jawbone and the subperiosteal implant. Sometimes a CAT scan is taken of the patient’s jaw which then allows the subperiosteal implant to be made without first exposing the jawbone which would otherwise require a two stage surgical operation.

Artificial Bone Substitute

This is a synthetic material which has similar properties to natural bone and the cost of dental implantswill cling to the live jawbone and grow like real bone. This type of implant is usually used as a foundation for dentures.

Endosteal Implants

Endosteal implants are similar to subperiosteal in that they are moulded or shaped to fit in a cavity in the jaw rather than sit on top of the jaw.

This article is intended only to give a very basic outline of the types of dental implants considered by the perodontist or oral surgeon. Sometimes a variation or combination of the types described may be used but it is important that if you are considering this type of dental treatment that you consult your dentist or a perodotist to establish what is the most suitable type of treatment for your unique situation.

Dental implants are considered by insurance plans as major dental work. A dental implant is really a metal screw that is placed in the jaw bone. It holds a false tooth or set of false teeth. They are similar to dentures or bridges, but they are permanent. Once they have been placed in the mouth, they act like a natural set of teeth.

Implants are expensive. Dental insurance plans usually only cover around fifty percent of the cost of the procedure. However, many dental plans will not cover implants at all. The cost of implants varies greatly as well. The cost of a single implant can be as low as five hundred dollars. It could also be as high as six thousand dollars. On average, implants costs between $1250 and $3000. In most cases, a patient will need more than one implant done and this causes the cost to rise dramatically. If a complete upper or lower set of implants is needed, the cost could be $15,000 to $30,000. A full restoration of teeth could cost as much as $80,000.

Why are implants so expensive? They require a tremendous amount of work by the dental team. Implants require a great deal of time spent by the dentists in both planning for and executing the implant procedure. An extensive evaluation will be done for each patient. The materials used in implants are expensive on their own. There are also additional costs associated with laboratory fees. Dental implants are not a quick, simple procedure.

As with any type of surgical procedure, there are risks associated with implants. Patients need to be good in health before undergoing dental implants. Healing time after the surgery can take three to six months. Patients who get dental implants need to be able to take good care of their oral health after the surgery.

Patients who are considering dental implants should discuss with their dentist whether they would make a good candidate for the surgery. They also need to decide whether the cost of the implants is worth the results. Some people feel that dental implants are far superior to dentures or bridgework. They want something that is more permanent. With implants, patients do not need to worry about their teeth coming loose. Other patients are willing to put up with a little more inconvenience in order to avoid the high cost of implants. The decision to get implants should be one that a patient makes with their dentist after considering all about dental implants,  the pros and cons.

Gingivitis – Periodontal Disease – Some Things You Should Know

Gum disease encompasses a wide variety of conditions, most commonly seen are gingivitis and periodontitis. Periodontal disease is a chronic bacterial infection that affects the gums and bone supporting the teeth. The disease may be limited to one tooth, or all the teeth. The disease takes hold when bacterial plaque on the teeth causes the gums to be inflamed. Plaque is the sticky film that forms on your teeth; it is like glue that helps the bacteria to stay on your teeth.

Gingivitis is the mildest form of gum disease and is totally reversible with professional dental cleaning and good home hygiene. Good home hygiene is not something people know how to do automatically. Most people need to be shown what to do so that they can get the most benefit from their brushing. Even people who brush well can benefit from some coaching. Even the best sports athletes in the world have coaches. The dentist and dental hygienist are tooth brushing coaches.

Gingivitis that is left untreated can advance to periodontitis. This happens when bacterial plaque spreads below the gum line. Toxins in the plaque cause a chronic irritation in the gums and bone starting an immune response. This immune reaction causes the gums and bone to break down and be destroyed. As the bone around the teeth is destroyed, the space between the gums and teeth deepens and the disease progresses even faster with more destruction. The early stages of the disease often have mild symptoms, so many people are unaware of a serious problem developing and do not seek the help of a dentist.

Signs of gum disease are:
Bright red, swollen, or tender gums
Bleeding gums (gums should not bleed when you brush or floss)
Shrinking gums
Loose teeth
Pus from around the teeth
Bad breath that does not go away
Change in the position of the teeth

Though the main cause of periodontal disease is bacterial plaque, other factors make it more likely for you to get this disease. First and foremost is tobacco use. Smokers have more gum disease and are more likely to loose teeth. Moreover, they do not respond as well to treatments for gum disease. Smoking is also a risk factor for developing serious oral cancer, as well.

Other risk factors for periodontal disease are family history, stress, medications, clenching and grinding, diabetes, poor nutrition, and various systemic diseases.

Risk factors that increase the likelihood of getting gum disease have been known for a long time, however, in recent years, gum disease as a risk factor for other medical conditions has become clear. Recent research by Marjorie Jeffcoat, D.M.D., the dean of University of Pennsylvania School of Dental Medicine, has shown that scaling and root planing resulted in an 84 percent reduction in premature births in women who were less than 35 weeks pregnant. Scaling and root planing is a common gum procedure to clean the teeth above and below the gum line and poses no risk to the mother and unborn child. Premature birth is a significant problem in the United States with nearly 12 percent of the babies being born preterm (before 37 weeks of pregnancy). Preterm birth results in increased death and other long lasting problems for these babies.

Before becoming pregnant all women should seek a gum and tooth exam as a way of avoiding this serious complication of pregnancy. For women who are already pregnant when gum disease is detected, scaling and root planing can still help. This treatment should probably be carried out in the second trimester.

Treatment for gum disease comprises many different modalities. Most common is scaling and root planing to clean the hard and soft deposits above and below the gum line. At the same time the dentist may use various antimicrobial agents placed in the space between the gums and tooth to help fight the bacteria and improve the healing. For patients whose periodontitis has altered the gums and bone too much, various surgical therapies are often applied. Depending on the nature of the disease, new bone and gums may be grafted in localized sites to improve the condition. For patients whose tooth supporting bone loss is so great that they loose a tooth, dental implants may be an option to replace what they have lost.

Some treatments for the gums focus on cosmetics. These procedures are often called oral plastic surgery. Patients with a gummy smile (a display of too much gum when they smile) can be helped by reshaping the gums. Patients with an irregular line of gums, or with gum recession on individual teeth can be helped with various techniques to restore missing gum tissue. This often has not only a cosmetic benefit but, also helps to cover exposed root surfaces which are painfully sensitive to foods and cold liquids.

In conclusion, modern medicine has helped us all to come to expect an ever longer life. Good dental care, which includes attention to the condition of the gums, is one important part. Regular tooth and gum check-ups can help prevent a wide variety of conditions and help us to retain our teeth for a long productive life.

A quick and short article. Warnings and what to expect when cosmetic dentistry education classesgetting your teeth whitened professionally. This doesn’t happen to all people apparently, but it’s good to watch out for. My dentist sent me home with some Day White whitening gel. I was pretty excited, I thought it would be easy, fast, and effective. But what I found out was actually something my dentist must have "forgotten" to mention.

Here’s the story.
I put the Day White whitening gel into my retainer as I was told. As SOON as this stuff touched my teeth, it hurt. That instant, it got onto my gums, and felt as if it was eating away at them. Which later I 

 

came to realize was true. Then about forty minutes after experiencing very uncomfortable pain I felt a sharp shoot of pain in my back molar.

I started thinking to myself, "What?! What’s going on!"

So I researched.

What I found out is that other people experienced this too. Sure, it whitens the teeth, but it eats away at the enamel and makes your teeth very sensitive for quite some time. Also It has been said that when you have it on for the full hour and a half, for however many days, that the sharp pains from inside your teeth last for days after.

 

 

A very uncomfortable, painful experience. I read that some people stayed in bed for days because they couldn’t handle the pain. It also bleached my gums for about a half hour, and later I found my gums were inflamed. After I felt that first sign of pain, and looked it up, I took them right out and said, "It’s not worth it." That’s how intense the pain was. Now I don’t know anything about the store brands, so don’t think that’s what I’m talking about. But it might be good to look into that before you buy. So you must ask yourself before make the decision to whiten, "Is it worth it?

In this article I would like to talk about another tip on how you can promote professional developmentthe growth of your business in your dental practice and earn more money. This is something we focus specifically at our dental practice growth website! In my previous articles I have talked about identifying your top5 to top 10 goals, making a list of action steps for your No.1 goal so as to start getting yourself to move in a direction and take action on it right away, then you identify and get mentors who can help you achieve your goals, and then identify your CLF (or your Critical Limiting Factor) which is the one thing that is completely holding you back with whatever it is you want to accomplish and do. These factors are very important in promoting the growth of your dental practice!

In the growth of your dental practice, there are so many different components that could be holding you back. It could be your team. It could be your area. It could be your lack of knowledge in marketing or you’re trying to grow your business. But it doesn’t really matter. The reality is that only you can really identify after probing and asking good questions, and continually educating yourself. When you have identified your limiting factor, you would want to go work on that.

So in this article, I’m going to talk about creating the discipline of constant education of specialized knowledge. This is one of the very important factors that you should have as you try to promote the growth of your dental practice. Napoleon Hill in his book Thinking And Growing Rich talked about how the greatest people, the wealthiest people of their time have what he called specialized knowledge. They weren’t average at many, many different things. They became really, really good and knowledgeable about key things that the majority of the public aren’t, or the majority of the people within their industry are not experts in. Earl Nightingale also said a long time ago that an absence of a mentor or model to follow for a new business owner can look at what 97% of the population or the average or the herd is doing and then create a plane based on doing everything that is opposite of what the traditional people are doing. And so its the same thing with knowledge; the easiest way to get to the top of the profession or income ranks is to take a look at what the people who are making the average income are doing, and then you do something totally different or do something precisely focused on where the money is or where the opportunity is in order to get your biggest bank for the time invested and the money that you’re willing to put into it.

So at this point in time, what I want you to do is think about this: ‘What is my Critical Limiting Factor and what is the knowledge that I need to acquire?’. And then now we want to create a gameplan and a discipline of minimally one hour a week to use focus and educate yourself on that specific topic. Now if you’re more aggressive like myself, minimally you can have 30 minutes a day on that topic. And then you’ll be amazed with the results! You can turn your driving time into learning time. You can turn your morning breakfast coffee or your evening before you go to bed into those preparation time so that you can really make the most out of each and every day and move forward into becoming what we could call a top 1 percenter! So go take action, and promote the growth of your dental practice!
